This case report describes a 72-year-old patient (Patient T) who became the first to undergo chyme reinstallation between two distant stomas using The Insides® Channel device. Following elective open repair of an abdominal aortic aneurysm, the patient experienced distal embolic complications resulting in patchy visceral necrosis. Emergency laparotomy and segmental bowel resection led to the formation of three stomas, including a high-output ileostomy and two colostomies representing the unused colon. The Insides® Channel enabled reinfusion of chyme into the distal colon, promoting functional use of defunctioned bowel segments. This intervention supported nutritional absorption, maintained mucosal integrity, and facilitated a successful later reconnection of the ileostomy and ascending colon.
